Alvin Stafford is a 63-year-old white male with a 94 pack-year smoking history who presents with 2 days' duration of worsening dyspnea on exertion, cough productive of yellow-green sputum, wheezing and poor appetite. Current symptoms are superimposed upon chronic, progressive dyspnea, exercise intolerance, and morning productive cough (clear). Physical exam is significant for low-grade fever, tachypnea, distant breath sounds, and expiratory wheezing. Mild diffuse clubbing is present; no cyanosis or peripheral edema.
