M.K. is a 34 yo F that presents with dysuria, fever and chills and uncontrolled hyperglycemia for the past several days. She has evidence of dehydration in her dark colored urine, dry oral mucosa and tenting when checking skin turgor. She is rating pain as 9/10 with no relief. She has a hx of kidney stones in the past, a new sexual partner for the last 3 months with whom she states using protection against STI's, and hx of lack of preventative care screenings and vaccines. On exam she has suprapubic pain, right sided flank pain, tachycardia, is febrile, and positive for symptomatic orthostatic hypotension. She reports a poor appetite and general feeling of unwellness.
